This is the ONLINE intake appointment for new clients who are not located in Wisconsin. This session is 90 minutes.
Please go to the left side of this page where you can download and complete the necessary paperwork. Completing this prior to your appointment will allows us to have more time to talk during the session. Paperwork may be e-mailed to jay@awentherapy.com prior to your scheduled appointment. It may also be sent by USPS but must be sent with sufficient time to arrive before your appointment.
Online sessions take place via SecureVideo conference service. Within 24 hours of scheduling you will be receive an email with instructions for using the service. Your computer must have a camera and microphone.
Payment for online sessions must occur prior to the session. Checks may be mailed with sufficient time for receipt. Payment may also be made over the phone by credit card or online by PayPal. Payment instructions will be emailed after your appointment is scheduled.

Payment may be made by cash, check, Visa, MasterCard or Discover. Fees for services are payable at the time services are delivered unless other arrangements are made with the therapist.
I currently have made the choice to accept only fee-for-service. By paying directly for psychotherapy, the client ensures complete privacy as insurance would require that your information become part of your health record. The client also retains more control over his or her treatment in terms of duration and frequency as most insurance companies allow only a specific set of sessions based on a diagnosis for some course of treatment. By paying directly, a third party is not making decisions about your care: You are in control.
In addition, paying directly for services avoids any need to medicalize treatment or assign diagnoses unless absolutely clinically necessary. Unfortunately, insurance companies will not pay for psychotherapy unless you are diagnosed with an illness. I provide therapy to any and all people without the stigma of being considered “ill” to seek out a therapist for life’s inevitable struggles. I refuse to give someone a medical diagnosis simply to appease an insurance company.
